My wife is heavier, I am totally attracted to her, but the mechanical problem is that she has to hold her skin back for clitoral stuff to feel good.
The thing is her hands get tired, and I want to give her full service sometimes so she can relax. But with one hand either fingering her or holding the vibrator, I just can’t hold things open with the other one hand.
I think I could do it with oral, since that frees up the hand, but I’m not very good at it yet.

Have you tried having her lay flat on her back and spreading her legs as far as she can manage while you kneel on the floor and eat her out or play with her? Pillows underneath to prop her up a bit? Maybe even try doggy style and go underneath her with the vibrator.
There happen to be some lip spreader implements you can use. Look at some sex toy shops.
