For context, I am a 24 year old trans guy who has been having this problem for about 4-5 years now (possibly longer but I never… touched myself intimately down there until I was 20-21 (that sounds pretty hard to believe but it’s the honest truth).

I am by no means a dirty person, and I like to think I take good care of my body. I wash thoroughly, and make sure to pull the hood back when I do wash/wipe. But I still get smegma. I’m not sure what to do, and I’m too embarrassed to talk to my doctor/a medical professional about my problem. Does anyone experience anything similar? I never even let my sexual partners give me head because even though I make sure to get rid of the smegma when I can, it still builds up despite me cleaning myself properly. I tried looking up how to fix the problem on google and it just tells me what I’ve already been doing, but it hasn’t been working at all. There’s a smell and it’s… unpleasant. I hate how gross I feel.

Edit: I should also mention I do not have any STIs.

  1. Well, if you are cleaning yourself but it’s still a big problem you really should try speaking to a doctor. I know it’s embarrassing, but there is a non-zero chance you may have an infection or something similar. Not necessarily an STI, but there are other bacterial or even fungal infections that could be a cause.

    If you are on hormones it could be affecting things too. If just cleaning yourself isn’t working, unfortunately a doctor is really your best bet.
