Is it a good system to have somebody appointed to an executive government department (ie; State Dpt or Dpt of Defense), or should there also be a Chief Executive to run the day-to-day?

Some countries have both. One difference is your appointed Secretary often has expertise around the department. In my country, this is what the CE would be, with the Minister being a higher ranking politician who receives advice from the Ministry.
