My semester is almost over and I didn’t get to pick any of my electives since I joined late. Assumed they were picked per semester which is not the case so I emailed my counselor asking if I could change them. She told me I could fill out a form yet didn’t give me the form (digital form) and I responded asking for clarification on something else + for the form but, it’s been 3 days and she hasn’t responded (responded within the hour for my first email) and I’m worried that it’ll be to late. Semester ends on the 20th so I don’t have much time. I want to send a follow up but, don’t want to sound rude. Is this okay? Any pointers?

I just wanted to send a follow up email since the semester is almost over and I did manage to find out what classes I’m taking next semester; and I’m not a huge fan of them. So I’d really like to fill out the form yet; I can’t find the form anywhere so if possible could you please send it to me?
Don’t mean to pester you for a response and understand if your busy but, like I said semesters almost over and I really don’t want to do one of the eletives in particular and want to atleast try to do something else.
